Antibiotic resistance (AR) in nosocomial infections presents a significant challenge to healthcare systems globally, with Pakistan experiencing alarming increases in multidrug-resistant organisms. This review synthesizes current evidence on the epidemiology, molecular mechanisms, and clinical implications of AR in hospital-acquired infections in Pakistan. It highlights emerging threats from pathogens such as carbapenem-resistant Enterobacteriaceae and methicillin-resistant Staphylococcus aureus (MRSA). The article further discusses policy interventions focusing on antimicrobial stewardship, infection control practices, and national regulatory frameworks aimed at mitigating the AR burden. Comprehensive surveillance and coordinated policy responses are imperative to curtail the spread of resistance in healthcare settings.
