Regenerative medicine integrates stem cell science, tissue engineering, and translational manufacturing to restore, replace, or augment lost function. Rapid advances in induced pluripotent stem cells (iPSCs), adult stem cell–based therapies, and biofabrication have accelerated clinical development across hematology, ophthalmology, orthopedics, cardiology, and dermatology. Converging technologies—bioprinting, organoid platforms, gene editing, and smart biomaterials—are enabling precise microenvironments and scalable production under current Good Manufacturing Practice (cGMP). Meanwhile, risk-based regulatory pathways (e.g., ATMPs, RMAT) and standardized characterization/potency assays are shaping evidence generation. This article reviews cell sources and mechanisms of action; scaffold, hydrogel, and bioprinting strategies; manufacturing and quality frameworks; clinical evidence and indications; and ethical, safety, and equity considerations. We synthesize emerging trends such as combination products (cell + gene + scaffold), digital twins for process control, and immune-informed design. We conclude with a roadmap for rigorous clinical translation emphasizing analytical comparability, long-term safety registries, equitable access, and interoperability of data and manufacturing standards.
