Rural populations in Pakistan continue to suffer disproportionately from chronic diseases due to limited access to healthcare infrastructure and specialist services. Telemedicine has emerged as a promising solution to bridge these gaps by providing remote healthcare access and continuous monitoring. This study evaluates the impact of telemedicine on chronic disease management—specifically diabetes, hypertension, and cardiovascular conditions—within rural Pakistani communities. Through a combination of literature review, case studies, and analysis of implementation programs such as Sehat Kahani and Doctor, the article demonstrates improvements in patient compliance, reduced hospital visits, and enhanced clinical outcomes. However, infrastructural challenges, digital literacy, and policy constraints remain significant barriers. The study concludes with recommendations for sustainable telemedicine integration tailored to rural healthcare dynamics.
